You can’t out-eat a bad night’s sleep

Why metabolic health depends on more than what is on your plate

At The Noakes Foundation, we spend a great deal of time talking about food…and for good reason. What we eat has a profound effect on blood glucose, insulin resistance, weight, inflammation, and our risk of developing chronic disease.

But nutrition does not operate in splendid isolation. The body is not a collection of separate departments, with food reporting to one floor, sleep to another, and stress quietly filing paperwork in the basement. Everything is connected.

A new study published in Nature offers a powerful reminder that sleep is not simply the period when we stop answering emails and hope tomorrow’s problems sort themselves out. It is an active biological process with consequences that reach across both brain and body.

What did the researchers investigate?

Researchers from the MULTI Consortium examined the relationship between sleep duration and 23 measures of biological ageing. Rather than looking only at the brain, they used medical imaging, proteins and metabolites to estimate ageing across multiple organs and biological systems.

The main analysis drew on data from the UK Biobank, which includes around 500,000 middle-aged and older adults. Participants reported how many hours they generally slept in 24 hours, including naps.

The researchers found a broadly U-shaped relationship: both shorter and longer sleep were associated with signs of greater biological ageing. Across the biological systems showing significant associations, the lowest estimated biological age gaps occurred at sleep durations ranging from approximately 6.4 to 7.8 hours, depending on the organ system and sex. 

Compared with sleeping for six to eight hours, sleeping for less than six hours or more than eight hours was also associated with a higher risk of several diseases and death from any cause. Short sleep showed particularly wide-ranging associations, including with type 2 diabetes, obesity, high blood pressure, heart disease, depression and anxiety.

That does not mean that seven hours and thirteen minutes is a magic number, or that sleeping in on Sunday has suddenly aged your pancreas. The ranges differed between biological systems and individuals, and sleep duration is only one aspect of healthy sleep. Quality, timing, regularity and conditions such as sleep apnoea matter too.

Long sleep also needs careful interpretation. Sleeping for longer may sometimes contribute to poor health, but it can also be a sign that illness, depression, medication or another underlying problem is already present.

Metabolic health is a team sport

The study focused on sleep, but its broader message is highly relevant to metabolic health: our daily behaviours interact.

Poor sleep can affect appetite, food choices, glucose regulation and the motivation to exercise. Chronic stress can interfere with sleep and make convenience foods considerably more persuasive. Too little physical activity can reduce insulin sensitivity, while regular movement can support glucose control, muscle health, mood and sleep quality.

This does not make nutrition less important. It makes the full picture more important.

A nutrient-dense, whole-food eating pattern remains one of the most powerful tools available to support metabolic health. However, it should sit alongside other foundations:

  • getting sufficient, regular and restorative sleep;
  • moving regularly and maintaining muscle strength;
  • managing ongoing stress and allowing time for recovery;
  • avoiding smoking and limiting alcohol;
  • spending time outdoors and supporting a healthy body clock; and
  • maintaining meaningful social connection.

We do not need to perform all of these perfectly. Health is not a school report in which one poor mark means repeating the year. But we do need to recognise that eating well cannot always compensate for sleeping badly, sitting all day, living under relentless stress or relying on alcohol to switch off.

What can we take from the study?

The practical message is not to become anxious about sleep (which, inconveniently, is not a very effective way to improve it!). Instead, we should treat sleep as part of healthcare rather than as the leftover time at the end of the day.

Simple measures can help: keep consistent sleep and wake times, seek natural light in the morning, reduce bright light and stimulating activity late at night, limit late caffeine, and create a bedroom that is dark, quiet, and comfortable. Persistent snoring, gasping during sleep, insomnia, excessive daytime sleepiness, or an unexplained need to sleep for unusually long periods should be something you speak to your doctor about.

This study adds to an increasingly clear picture: sleep is not an optional extra for the brain while the rest of the body waits outside. It is woven into the biological systems that influence how we age and how disease develops.
