Overview

Real-World Nutrition and Metabolic Health Research investigates how nutrition, lifestyle and metabolic-health interventions work when they move beyond theory and are tested in the complexity of everyday life. Building on Prof Tim Noakes’ pioneering work questioning conventional assumptions about nutrition, carbohydrate intake, insulin resistance and metabolic health, The Noakes Foundation investigates these questions across communities, clinical populations, healthy adults, athletes and collaborative research settings.

Our work combines clinical and metabolic measurements with behavioural and qualitative evidence. Rather than asking only whether an intervention produces a measurable effect, we investigate what works, why it works, for whom, under what circumstances, and whether it can realistically be implemented and sustained.

Nutrition research is often dominated by numbers: body weight, glucose, cholesterol, blood pressure and other biomarkers. These measurements are essential, but they cannot explain the full impact of an intervention. People eat within families and communities. Their food choices are influenced by affordability, culture, beliefs, work, social relationships, healthcare access and their previous experiences with food and health.

An intervention may improve clinical outcomes while still being difficult to maintain. Conversely, participants may experience important changes in hunger, energy, confidence, wellbeing or their relationship with food that are not adequately captured by traditional clinical measurements. The Noakes Foundation (TNF) and Eat Better South Africa (EBSA) therefore makes extensive use of mixed-methods research, integrating quantitative clinical and metabolic outcomes with interviews, focus groups and behavioural data. This allows us to investigate not only whether change occurred, but also how participants experienced that change, why it occurred, and what helped or prevented them from sustaining it.

This research theme has grown from community-based nutrition work into a broader portfolio encompassing metabolic and clinical research, continuous glucose monitoring and digital health, sports and food science, and collaborative studies with academic, healthcare, philanthropic and industry partners. The research is organised around three interconnected areas of contribution.

1. From Nutrition Education To Sustainable Community Change

Eat Better South Africa: Evaluating Nutrition Education in Under-Resourced Communities

Why is this important?

Knowing what constitutes a healthier diet does not necessarily mean that people can follow it. Food choices are influenced by affordability, food availability, household responsibilities, culture, family expectations, health literacy and the wider socioeconomic environment. These factors become particularly important in communities simultaneously facing food insecurity and a high burden of metabolic disease.

Community nutrition research therefore needs to begin by understanding the community itself. Through Eat Better South Africa, TNF has investigated how low-carbohydrate, healthy-fat and real-food nutrition education can be adapted, delivered and evaluated in under-resourced South African communities. Our work has progressively moved from understanding participants’ perceptions and community environments to evaluating dietary behaviour, metabolic health and longer-term implementation.

It has also expanded to include healthcare professionals and health services, recognising that sustained community health improvement requires engagement beyond individual participants. Current EBSA research has extended this work into people living with type 2 diabetes and other comorbidities, including one-year follow-up and engagement with healthcare practitioners and public-health stakeholders at community clinics in Dunoon, Vanguard and other under-sourced communities in the Western Cape.

The Real Meal Revolution helped translate the principles of carbohydrate restriction and real-food nutrition into practical guidance for the general public. Its success raised an important subsequent research question for TNF: can this type of dietary advice be successfully adapted and implemented across communities with very different social and economic circumstances?

This work extends real-food principles into the family environment, recognising that dietary behaviours develop within households rather than in isolation. This provides an important foundation for the Foundation’s growing interest in family, caregiver and child nutrition and health literacy.

User perceptions of continuous glucose monitoring during high and low carbohydrate diets in trained, non-diabetic athletes | Science & Sports, 2026 | DOI: 10.1016/j.scispo.2026.03.003

This mixed-methods crossover study examined how highly trained, non-diabetic athletes experienced continuous glucose monitoring during contrasting low-carbohydrate high-fat and high-carbohydrate low-fat dietary interventions. It demonstrates that CGM research should consider not only glucose values but also how users interpret and respond to their own metabolic data.  This work helped establish an important question for TNF’s developing digital-metabolic research: what does CGM add to our understanding of nutrition when it is used outside conventional diabetes care?

Experiences and behavioural responses associated with continuous glucose monitoring among healthy adults following a low-carbohydrate high-fat diet and intermittent fasting: A mixed-methods exploratory study

This study extends TNF’s CGM research from trained athletes to healthy adults following a low-carbohydrate, high-fat diet and intermittent fasting. By combining continuous glucose data with participants’ experiences and behavioural responses, it investigates not only what glucose patterns occur but also how seeing real-time metabolic information may influence awareness of food, fasting and individual physiological responses. The study therefore considers CGM as both a metabolic measurement technology and a potential behavioural tool, while also helping identify the limitations of interpreting CGM data among people without diabetes. The August research update records this CGM mixed-methods programme among TNF’s completed/current research outputs.

Low-carbohydrate nutrition in hypothyroidism: a 12-month mixed-methods case study. Journal: African Journal of Primary Health Care & Family Medicine

This mixed-methods study examined the longer-term effects and experiences of carbohydrate restriction in women with hypothyroidism, with follow-up at six months and one year. Women completed two online nutrition courses and took part of several group discussions throughout one year. It contributes real-world evidence on how a low-carbohydrate dietary approach may influence metabolic health over time in a population that is relatively underrepresented in nutrition intervention research.

2. Translating Nutrition Science Into Clinical, Digital And Metabolic Evidence

Why is this important?

Nutrition recommendations ultimately need to demonstrate whether changing what people eat produces meaningful and measurable changes in health. This part of TNF’s work moves from nutrition education and behaviour into clinical, metabolic and digital investigation.

Depending on the research question, studies may examine glucose regulation, body composition, weight and waist circumference, blood pressure, lipid profiles, inflammatory markers, endocrine outcomes, mental wellbeing, exercise physiology and other markers of metabolic health.

The aim is not to assume that a dietary approach is effective simply because it fits a theoretical model. Instead, nutrition and lifestyle interventions are subjected to empirical testing in different populations and circumstances.

Continuous glucose monitoring (CGM) has become an increasingly important part of this research. CGM allows glucose responses to be observed continuously during everyday life rather than relying solely on isolated clinical measurements. TNF’s research also examines the human response to the technology: how people interpret their glucose data, whether seeing those data affects dietary behaviour, and how metabolic responses differ according to diet, physical activity and individual characteristics.

This creates opportunities to move beyond population averages towards a more personalised understanding of metabolic response. TNF’s current portfolio now encompasses CGM, diabetes, thyroid health, intermittent fasting and exercise/metabolic physiology, demonstrating a substantial expansion beyond conventional nutrition education alone.

Prof Noakes places insulin resistance and diet-related metabolic disease at the centre of discussions around chronic disease and translates these concepts into practical nutritional strategies. This early EBSA research placed participants’ voices at the centre of programme evaluation. It explored why women participated, how they experienced the intervention and which practical and social factors helped or hindered dietary change.

This research demonstrated why communities should be understood before, rather than only after, interventions are implemented. This research addressed an important challenge in nutrition science: people do not always accurately report what they eat. By comparing Eat Better South Africa’s participants reported dietary intake with actual supermarket purchasing decisions, the study demonstrated how behavioural measures can strengthen conventional dietary assessment and provide a more complete picture of whether nutrition education translates into real-life food choices.

Mixed methods evaluation of a low-carbohydrate high-fat nutrition education program for women from underserved communities in South Africa | Appetite, 2025 | DOI: 10.1016/j.appet.2024.107725

This study demonstrates the value of integrating clinical and qualitative evidence. Metabolic improvements occurred alongside important Eat Better South Africa participant’s experiences relating to hunger, energy and health, while qualitative findings identified social, economic and environmental barriers to maintaining dietary change. Together, the findings provide a more complete explanation of intervention effectiveness than either quantitative or qualitative evidence could provide alone.

This study extended the EBSA research programme beyond community participants to the healthcare professionals who advise and treat them. It highlighted differences in nutrition knowledge and attitudes towards LCHF, alongside concerns around affordability and sustainability, illustrating some of the challenges involved in translating emerging nutrition evidence into routine healthcare practice.

3. Building Evidence Through Research Partnerships

Why is this important?

Important nutrition questions often sit at the intersection of science, public health, food systems, healthcare, technology and commercial practice. Food producers, healthcare organisations, technology companies, professional bodies and other organisations may have access to populations, products, technologies or practical questions that create valuable opportunities for scientific investigation.

These collaborations can allow researchers to test assumptions about foods, health interventions and consumer behaviour that might otherwise remain unanswered. However, the scientific value of such partnerships depends on maintaining a clear distinction between research and marketing.

TNF’s role is to take relevant real-world questions and subject them to appropriate scientific investigation. Depending on the question, research partnerships may combine qualitative investigation, consumer behaviour, dietary interventions, clinical measurements, CGM and mixed-methods evaluation. Where external organisations support or commission research, TNF’s contribution remains centred on scientific rigour, transparent reporting and independent interpretation of the findings.

This has become an increasingly important part of TNF’s research portfolio. Existing work includes SAPPO, Triba and egg research, while potential future studies extend to red meat, nuts, breakfast foods and other questions concerning food choice and metabolic health.

Salient TNF research outputs

This research investigates how strength and endurance athletes understand pork, protein, dietary fat and sports nutrition and explores how nutritional knowledge, sporting culture, personal beliefs and practical considerations influence food choices. It provides evidence for designing nutrition interventions around how athletes actually understand and use foods, rather than assumptions about athlete eating behaviour. TNF’s SAPPO programme has subsequently progressed into a mixed-methods intervention examining nutritional education and the use of pork as a primary protein source among endurance- and strength-trained athletes.

From Perceptions to Practice: Anthropometric, Metabolic, and Experiential Outcomes of a Pork-Based Nutrition Intervention in Athletes: A Mixed-Methods Study. Nutrients.

This mixed-methods intervention study examines how nutrition education and the inclusion of pork as a primary protein source influence dietary behaviour, health and nutrition-related outcomes in strength- and endurance-trained athletes. By combining quantitative outcomes with participants’ experiences, the study provides real-world evidence on the role of whole-food protein sources within athlete nutrition and how education may shape food choices and perceptions.

Feasibility and Metabolic Outcomes of a Digital Lifestyle Intervention for Type 2 Diabetes in South African Managed Healthcare Journal: Journal of Metabolic HealthContribution.

This research independently evaluates the feasibility, acceptability and effectiveness of a digital diabetes reversal programme within a South African medical-scheme setting. Rather than relying solely on programme-provider outcomes, it contributes independent scientific evidence about how a commercially delivered digital metabolic-health intervention performs when implemented in a real healthcare environment. Effects of Breaking a Fast with Eggs on Metabolic and Glycemic Outcomes During a Low-Carbohydrate, High-Fat Diet with Intermittent Fasting: A Pilot Randomized Controlled Trial

This research independently evaluates the feasibility, acceptability and effectiveness of a digital diabetes reversal programme within a South African medical-scheme setting. Rather than relying solely on programme-provider outcomes, it contributes independent scientific evidence about how a commercially delivered digital metabolic-health intervention performs when implemented in a real healthcare environment. Where our research is going next

TNF’s research programme continues to evolve from evaluating individual nutrition interventions towards understanding how nutrition and metabolic-health approaches can be personalised, digitised, culturally adapted and implemented across different populations and settings. Future directions include longer-term community follow-up, digital delivery of EBSA, further CGM research, personalised metabolic monitoring, family and child health, international adaptation of community interventions, and new academic, healthcare and industry research partnerships.

The proposed digital development of EBSA, for example, aims to combine accessible nutrition and health education with personalised coaching, peer support, health monitoring and evaluation, potentially allowing the programme to reach larger populations while maintaining evidence of engagement and effectiveness.  Emerging precision-metabolic research also creates opportunities to integrate CGM, dietary intake, physical activity, body composition, clinical information and data-driven modelling to understand why individuals may respond differently to the same nutrition or lifestyle intervention. The OPEN-METAB proposal represents this direction and identifies TNF/EBSA as a potential environment for future international validation and transfer.  Together, these areas continue to address the central question underlying TNF’s research:

What happens when established ideas about nutrition and metabolic health are rigorously tested in the people, environments and circumstances in which they actually need to work?
